Creates and promotes opportunities and systems to enhance improved outcomes across the continuum of healthcare services.
1. Primary responsibility includes clinical coordination, resource utilization, pathway tracking, and discharge planning for medically complex patients 2. Serves as liaison among all disciplines. 3. Facilitates communication and access to care. 4. Evaluates patient's formal and informal support systems. 5. Seeks appropriate levels of care for patients to include home, assisted living, convalescent care, or designated medical facilities 6. Educates patient, family, and community support services to facilitate the goal of self-care by the patient and family 7. Monitors patients progress toward goals; facilitates discharge when patient is medically stable and appropriate for release. Arranges transportation and obtains pre-authorization for ambulance, wheelchair van, lifeflite, etc. 8. Coordinates crisis interventions on identified patients to include communicating with the patient, family, Crisis response team, administration and others as appropriate. 9. Participates with patient, family, physicians, patient care personnel, and other members of the management team in planning of patient care services and the patient care plan. 10. Facilitates resource management to enhance effectiveness, continuity and cost effective care and service to include utilization management, medical record review, Paasar level screening and third party review. 11. Review ICU cases daily with ICU RN. 12. Implement utilization review on each patient upon admission, review for appropriate level of care and patient status. 13. Discusses specific length of stay or admission criteria with physician and contracted agencies. 14. Communicate with third party payors by providing clinical information and treatment plan for purposes of certification of hospital services in accordance with department standard. 15. Communicates with pre-certification staff, admission staff, and billing staff as needed to convey information integral to pre-certification or billing to facilitate authorization of hospital service. 16. Coordinates the case management communication to ensure organizational awareness of available services and resources; serves as the primary liaison with external entities in the continuum of care, services and support. 17. Documents all activities on appropriate forms.
Required: â?¢ Bachelorâ??s degree (BSW) in Social Work Services required* â?¢ Minimum 2 yearsâ?? experience in related field required
Preferred: â?¢ Three or more yearsâ?? related experience
*BA degree in a related field is acceptable for associates hired into the position prior to June 30, 2017
