The Pharmacy Department is expanding the inpatient practice model to include involvement in a multi-disciplinary patient discharge process on internal medicine units. The new model will incorporate transitions of care pharmacist services to patients at high risk for readmission to the hospital.
Inpatient Pharmacy services currently utilize an integrated decentralized staffing model. Pharmacists practice on nursing units participating in order verification, provision of drug information, and clinical services including pharmacokinetics, anticoagulation management, automatic intravenous to oral switches, automatic renal dose adjustments and automatic therapeutic interchanges.
The addition of transitions of care pharmacist functions will include identification of patients at high risk for readmission using a validated assessment tool. Targeted patients will receive a thorough assessment of home medications by a patient care technician and will be followed by the floor clinical pharmacist daily to ensure resolution of medication related issues. Prior to discharge, the clinical pharmacist will work to ensure the patient’s discharge medication list is accurate and optimized and assist with any needed discharge medication counseling. Pharmacist interventions will be tracked and trended to demonstrate the value of pharmacist related activities in this setting.
