BASIC PURPOSE OF THE JOB Provides medication reconciliation services under the direct supervision of a pharmacist. This includes: obtaining and entering medication histories into the computer software, as well as preparing medications to be dispensed, medication stock maintenance, record keeping and medication billing. JOB REQUIREMENTS Minimum Education High School/GED Required Minimum Work Experience 12 months of previous experience required, hospital, retail or medication reconciliation experience preferred Registration with the TN Board of Pharmacy required within 30 days of employment, successfully passing the PTCB certification for CPhT status required within first year of employment, active CPhT preferred. Required Skills, Knowledge, and Abilities Basic computer and typing skills. Math skills. PHYSICAL REQUIREMENTS Note: Reasonable accommodations may be made for individuals with disabilities to perform the essential functions of this position. Medium lifting, pushing and pulling is required for 20-50 lbs occasionally, 10-20 lbs frequently and 10 lbs constantly to move objects. Sudden emergency situations have the potential for exposure to lifting or moving of up to 100 lbs. Frequent bending, walking, sitting, squatting, reaching and standing are required. Keyboard/computer use and/or repetitive motions may be required.
