This position provides telephonic support to families of babies receiving services through the neonatal intensive care until (NICU). You will have the ability to make a significant and positive difference in the lives of the families we serve. ***This is a Monday - Friday position working traditional business hours as a telecommuter *** Primary Responsibilities:Communicate directly with families of babies currently admitted or recently discharges from the NICU, physicians and community / governmental agenciesCollaborate with NICU nurses regarding barriers or challenges families are confronting and assist with solutionThrough direct telephonic assessment, assist family members to continue to function with their babies illness impacts their physical health, mental status and social roleAssist families in applying for Social Security Disability BenefitsAsses for social, economic and emotional factorAssist members and families in discharge planning where necessaryDevelop working knowledge of resources readily available to interdisciplinary team and familiesPromote effective utilization and monitoring of community resources and assist in optimization of available benefits including referrals to outside vendorProviding education on community, Federal and State resources
Required Qualifications:Masters in Social Work (MSW) with active and unrestricted social work license in the state of residence3+ years of clinical experience in a neonatal or NICU setting Very strong oral and written communication skills for a variety of audiences (families of babies in NICU, providers, community resource Knowledge of pertinent websites, community resources, and other ways to provide assistance to consumersProficient Computer skills in Microsoft Office such as Word, Outlook and InternetMust reside in the Central or Eastern Time ZonesPreferred Qualifications:Recent experience in discharge planningCertified Case Manager (CCM)Experience in Managed Care / Insurance (MCO)Knowledge and experience with Medicaid programs and regulations Careers with Optum.
