Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patient's age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering weekends and team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our patient caseload and the needs of the organization. Scope of Duties Professional nursing service is provided by a registered nurse and includes 1.Initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points 2.Initiating the plan of care and revising as necessary 3.Providing those services and/or treatments requiring substantial and specialized nursing skill 4.Counseling the client and family in meeting nursing related needs. 5.Initiating appropriate preventive and rehabilitative nursing procedures 6.Documenting clinical/progress notes 7.Coordination of services to include referrals to other services and informing the physician and other staff of changes in the client's needs 8.Evaluating outcomes of care 9.Supervising LPNs' and home health aides providing services 10.Participating in in-service programs 11.Planning for discharge from service 05/2011
