NICU On-Site RN Case Manager - Telecommute from Hartford, CT

As an On Site RN Case Manager, you'll wear many hats and work in a variety of environments. This position will be required to work onsite at Children's Medical Center in Hartford, CT from 2 - 5 days per week. When not working On Site at the hospital, this will be a telecommute / work from home position. You will also be required to work at other hospitals in the area as needed. The shift is M-F during normal business hours. If you are located in the greater Hartford, CT area, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities: Visit members in hospitals Provide a complete continuum of quality care through close communication with members via in-person or on-phone interactionSupport members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on WheelsAssist members with the transition from a care facility back to their homeThis role will be located On-Site at Children's Medical Center in Hartford, CT 2-5 days per week and you'll need to be comfortable dealing with a full range of hospital based environments. Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential.
Required Qualifications: Current, unrestricted RN license in the state of ConnecticutAccess to reliable transportation3+ years of experience as a RN with high risk level II or level III NICU focusComputer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applicationsDedicated space in the employee's home for a home office and access to high speed internetPreferred Qualifications: Case management experienceCertification in Case ManagementDischarge planning experienceHome care/field based case managementExperience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needsMake a difference in countless lives, starting with your own.
