The RN Case Manager will assess, coordinate, and implement a medical plan of care for children residing in selected Tarrant County homeless shelters. This individual will be charged with the development and coordination of a program which oversees the health care for designated homeless children treated in the Cook Children's Health Care System. This position will be responsible for the compilation of data to measure the impact of this program on homeless children and the possible justification for replication in other communities. This individual will perform an ongoing review of the health care management of inpatient homeless children and follow up to monitor compliance with the post discharge plan of care. The prn applicant will need to be able to work a full time schedule in order to orient to this position and will need to have the flexibility to work multiple days in a row when the full time staff member is out on vacation. The ideal candidate would also be able to work additional shifts as an RN Case Manager in the Medical Center after going through orientation for that role. All orientation is full time until the employee has become fully integrated into the designated role and can perform the position's duties independently.
Graduate of an accredited nursing program
Current Texas RN license required
Minimum of three years of clinical experience required
CPR - BLS required within 30 days of start
Previous experience in case management/utilization review preferred
