Take this opportunity to start doing your life's best work.(sm) This is an on-site position located at Children's Mercy Pediatric Care Network (PCN) office in Kansas City, MO for 2 days a week. The other 3 days will be working from home with expectation of Field Visits approximately 40% of the time meeting with beneficiaries in facilities, homes, etc. Field visits may occur up to 60 miles from you home. As a Behavioral/Mental Health Care Advocate you will be responsible for case management of behavioral health and substance abuse cases. You'll have a direct impact on the lives of our members as you recommend and manage the appropriate level of care throughout the entire treatment plan. Primary Responsibilities: Manage caseload of members with behavioral health related issues Conduct assessments and develop plans of care to improve member health outcomes Make both member and provider contacts to address identified care plan needs. Contacts will be primarily telephonic but will include some field and facility visits Provide clinical consultation to nonclinical staff on behavioral health related issues Work as part of an interdisciplinary team Meet all identified performance metrics
Required Qualifications: Licensed clinician with a Master's degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D. Licenses must be active and unrestricted in Missouri 2 years post-Masters experience in a related mental health environment At least 1 year of experience working with Pediatrics Proficient Microsoft skills (Word, Excel, Outlook) Reliable transportation for field visits Ability to work on-site at Children's Mercy Pediatric Care Network (PCN) office in Kansas City, MO at least 2 days per week Reside in Missouri Access to high speed internet and dedicated workspace for home office set upPreferred Qualifications: Inpatient experience Dual diagnosis experience with mental health and substance abuse Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients Managed care experience (MCO) Independently Licensed in the state of MissouriCareers with Optum.
