Dartmouth-Hitchcock (D-H) is seeking a Director of the Office of Care Management to join our team. The Director will demonstrate sound, decisive leadership skills, have five or more years of experience in coaching, mentoring, team building and exhibit emotional intelligence, strong interpersonal communication skills and strategic vision. This role will lead a strategic plan to unify a system of community group practices, affiliates and community partner resources, throughout Vermont and NH. Dartmouth Hitchcock strives to provide the highest quality of integrated patient care and experience. Qualified candidates will lead a promising team-base, patient-centered approach designed to assist patients and their support systems in managing medical conditions.
Provides system leadership and consultation in all aspects of Care Management, Language and Interpreter Services, Lifeline Services and the Medication Assistance Program.
Provides strategic and operational leadership and is responsible to design, integrate and coordinate hospital care management services across the system.
Responsible for the development, administration and supervision of Care Coordination, Utilization Review, and Social Services.
Ensures processes are aligned with the organizational strategic plan and meet or exceed regulatory and industry standards and that the operational plan is effectively communicated throughout the organization.
Develops and implements a model for care management in the organization that improves quality, safety, and service for our patients.
Master's degree in clinical or health related field required.
Five years in leading acute care based care/case management, utilization review, discharge planning or other closely related quality programs.
Strong leadership, communication, and computer skills desired.
Required Licensure/Certification Skills:
Licensed Registered Nurse in New Hampshire required.
Certified as an Accredited Case Manager, Certified Case Manager, or Case Management Administrator required.
