The Experienced Pediatric AIDS Chicago Prevention Initiative (PACPI) Enhanced Case Manager (ECM) is an integral role of the case management program. Under the guidance of the Senior Manager, the Experienced PACPI ECM supervises less experienced case managers, provides clinical and programmatic oversight and also actively works with clients and their families to help them cope with issues related to HIV/AIDS. The Experienced PACPI ECM is expected to provide encouragement to team members, communicate team goals and identify areas for new training or skill checks. This position also assists with hiring processes and new team member training and supports the Senior Manager and performs management duties when the Senior Manager is unavailable. General management responsibilities include communicating program goals and deadlines, answering team member questions, helping with team member problems and overseeing team member work for quality and guideline compliance. Furthermore, the Experienced PACPI ECM provides client-centered counseling for adolescents, women and children and must have a good understanding and background in maternal and child health, substance abuse care and treatment, perinatal HIV care systems, homelessness and public aid systems and the Department of Children and Family Services (DCFS).
Experienced PACPI ECMs are each assigned to a geographic area in Northeastern Illinois that is defined by the PACPI program. Experienced PACPI ECMs must have a vehicle for 80% local travel and will be reimbursed for documented mileage. This position is wholly funded by contractual grant funding which is renewed under provisions set by the grantor of the contract. Employment will be contingent upon the continued receipt of these grant funds and satisfactory job performance.

Partners with Senior Manager to conduct performance evaluations for less experienced case managers
Supports Senior Manager and performs management duties when Senior Manager is unavailable
Provides encouragement to team members, including communicating team goals and identifying areas for new training or skill checks
Assists management with hiring processes and new team member training. Answers team member questions, helps with team member problems, and oversees team member work for quality and guideline compliance
Communicates deadlines and program goals to team members
Carries a caseload of up to eight clients.
Provides client-centered counseling for adolescents, women, and children
Conducts initial assessments of basic needs, psychosocial needs, medical needs, and identifies linkages and referrals needed
Maintains daily contact with participants and their children to support adherence to care, performs home visits to pregnant women to ensure home safety and appropriateness for young children
Provides one-on-one care, referrals to substance abuse treatment, addresses health insurance coverage needs, assists with and/or arranges transportation, food and housing, daycare, paperwork and documentation as necessary
Assists mothers in preventing their children from becoming a part of the DCFS system and works directly with DCFS to prevent at-risk youth from entering the system by developing relationships with community providers as well as program participants.
Provide intensive services to women and their families in low income and high risk communities in Chicago and must be available 24/7 for all PACPI clients via phone.Must carry a 24/7 Perinatal Hotline pager 2-3 times per year.
Responsible for documentation of all levels of patient and family contacts for both PACPI and AIDS Foundation Chicago (AFC).
Required to complete documentation of case management encounters, referrals, goals and plans in the designated database (currently the Provide System).
Must attend AFC case management trainings, PACPI meetings and case management team meetings.
Sensitivity to and understanding of issues concerning HIV/AIDS, teen pregnancy, domestic violence, poverty, and chronic illness
Detail-oriented with the ability to act as a liaison between clients and service providers
Functional knowledge of computers in order to enter client data into web-based reporting system
Attention to detail and ability to prioritize independently
Ability to maintain confidentiality, problem-solve, reason and communicate effectively in both verbal and written form
Strong ability to handle high pressure situations
Must have a good understanding and background in maternal child health, substance abuse care and treatment, perinatal HIV care systems, homelessness and public aid systems, and the Department of Children and Family Services (DCFS)
Possesses an understanding of perinatal HIV transmission, prevention and treatment
Comfortable with frequent communication via phone and e-mail
High level of proficiency with time management
Education, Experience, and Certifications:
Bachelorâ™s degree required
A minimum of five years of related social services experience required
A minimum of 1 year supervising or overseeing the work of others preferred
Experience working with children and/or families from high risk, low income, urban backgrounds required
Technical Knowledge or Skills:
Functional knowledge of computers in order to enter client data into web-based reporting system required
Demonstrated effective leadership and teaching skills required
Strong knowledge of program subject matter required
Strong organizational skills preferred
Strong knowledge of Chicago social service resources preferred
Strong interpersonal skills and the ability to work both independently and as part of a team preferred
