The Stroke Program Coordinator is fundamental to the development, implementation, and evaluation of the Stroke Program. S/he is responsible for direct patient care and coordination of care and/or case manages for stroke patients based on current clinical practice guidelines across the continuum of care. As the expert nurse, the Coordinator participates in all stroke alerts to support the delivery of evidence-based acute stroke assessment, care and management. The primary responsibilities are direct patient care which includes nursing process and practice, education, professional development and customer service to drive quality, improve clinical outcomes, lead post-hospital follow up, and serve as a resource for community outreach. Using a multidisciplinary approach, s/he will lead, implement, and evaluate process improvement and implemented care pathways. The Stroke Program Coordinator will work collaboratively with all hospital and clinic personnel on relevant performance improvement projects. The Stroke nurse coordinator applies experience and judgment to determine the importance of a situation, sets priorities, and uses abstract thought to evaluate clinical situations. Demonstrates leadership skills and the ability to problem solve on a broad level. Facilitates critical thinking of staff when addressing patient care issues. Provides education and coordination of patient care specifically for the Stroke population. Participates in the development and review of policies, procedures and protocols of new and existing Stroke services. Position reports directly to the Stroke Program Manager. Demonstrates accountability for own practices as defined by the Nurse Practice Act. Demonstrates investment in improving the overall quality of care in the clinic/unit.REQUIRED -Minimum 3 years of recent critical care experience. -Possession of valid California RN license. -Graduation from an accredited school of nursing. -ACLS certification. -Pediatric Advanced Life Support (PALS) or obtain within 6 months of hire. PREFERRED -2 years of Neurology experience. -Current NIH Stroke Scale (NIHSS) certification. -Emergency Neurologic Life Support (ENLS). -Moderate Sedation certification. -Bachelors of Science in Nursing degree (BSN). SPECIAL REQUIREMENT -Must be able to work overtime as required. -Must be able to work flexible hours; day, evening, night, weekend, holiday shifts and on-call. -Must be able to wear radiation protection apparel weighing up to 25 lbs for extended periods of time. -THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT
