At Stony Brook Medicine, our clinical staff are critical members of the team who assist in diagnosis and treatment responses to actual or potential health problems by methods and techniques such as case finding, health teaching and counseling. Our valued team members provide care supportive to our patients' health and well-being, while adhering to a medical regimen.
Duties of a Diabetes Educator may include the following but are not limited to: :
Provide 1:1 DSME in the outpatient setting.
Responsible for communicating with referring physician.
Addressing issues that arise in the DSME outpatient. Setting.
Supports instruction in Diabetes Education for Nursing Orientation and Critical Care.
Primary instructor for group diabetes classes.
Responsible for the processing of bills for DSME services.
Work closely with DSME program coordinator and Endocrine team to meet program goals.
Active Member of the Diabetes Advisory Committee. Serve as a liaison between staff, advisory members and the facility administration.
Plan and participate in the orientation sessions for new diabetes patient education personnel when applicable.
Participate in and support Diabetes education of house staff (including residents and fellows as needed).
Participate in the preparation of the Budget for the diabetes patient education program.
Complete ongoing program evaluation, quality assessment and improvement in Diabetes Education program.
Follow up with individual and group patient participants as indicated.
Ongoing review and assessment of program resources.
Maintains program infrastructure; Responsible for program design implementation and overall success.
Supervision of clerical staff when appropriate.
Maintain and document patient records per standards of care and to meet certification requirements.
Responsible for Upkeep and ordering DSME Education supplies.
Reports to senior Diabetes Educator.
Required qualifications: License to practice in New York State, Bachelor degree, at least 5 years Med/Surg/Critical Care experience with specific focus on patient/family education in diabetes self-management.
Preferred: Certified Diabetes Educator (CDE) with experience teaching comprehensive diabetes self-management education. New York State Masters Preparation, 2-5 years' Administrative experience specializing in Diabetes program management and development, research experience, quality assurance experience, and previous regional/national presentations &/or publications.
