Provides patient and family focused care coordination for infants exposed to opioids prenatally or diagnosed with Neonatal Abstinence Syndrome (NAS) as a member of the Grow With Me Program Team. Collaborates with all team members to promote patient and family self management and to optimize the health and well being of the patient and family
Minimum Education: Master's degree in Social Work.
Minimum Work Experience: 2-4 years of related experience
Required Licenses/Certifications: Li censed Master Social Worker in the State of Tennessee.
Required Skills, Knowledge and Abilties: Strong computer literact including word processing. Excellent written and verbal communication skills. Strong level of self initiative with the ability to set priorities and make decisions of a complex nature. Ability to work and interact effective with a multi-disciplinary team.

Light lifting, pushing and pulling is required for 10-20 lbs occasionally and frequent moving of objects of less than 10 lbs is required. Frequent sitting with some walking, standing, squatting, bending and reaching is required. Keyboard/computer use and/or repetitive motions may be required.
