The Care Coordinator works closely with the primary care physicians, various specialty physicians, hospitals and ancillary service providers to maximize the health status of patients in their assigned primary care practices.
The Care Coordinator coordinates care and care plans across in-patient and out-patient settings, facilitates transitions of care for out of network patients and in conjunction with the practices, provides hospital follow-up for all patients.
Additionally the Care Coordinator provides high risk patients with chronic conditions health coaching and education and facilitating their compliance with the required quality metrics. These activities are provided telephonically and in a practice or home setting based upon predefined criteria and assignments. This position requires traveling between facilities and locations.
Registered Nurse with clinical experience and current, active and unrestricted new Jersey State License.
