Erie Family Health Center is looking for a motivated, caring and focused individual like you to join our Case Management Team! The Case Manager II will engage with clinical and supportive service teams to identify potential clients and will outreach, assess, advocate for and deliver supportive services to clients.
Provides case management services including; coordination of primary care (prenatal and/or pediatric care) and social services, outreach, face-to-face follow ups, risk assessments completion, referrals, home visitation, and tracking clients’ appointment attendance
Documents required data into electronic systems (EMR & Cornerstone).
Collaborates with fellow Case Managers to provide constant floor coverage so as to assure productive presence of a Case Manager in the health center throughout the day
Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up process
Completes and collects participant's surveys and screeners as required by grantors
Follow the Department of Human Services Maternal and Child code requirements
Mandated reporter to the Department of Children and Family Services
MSW Required in (social work, child development, community health or related field)
Minimum of two years' work experience in health, social or community services preferred
Bilingual (English/Spanish) (May be required to assist or communicate with non-English/Spanish only speaking patients and/or guests.) and bi-cultural competency preferred
Cultural competency to work in a low income, minority environment
Computer skills and time management skills required
Ability to maintain confidentiality and trust of clients
Self-motivated individual who can manage multiple projects and takes ownership over responsibilities
Local travel required, valid Illinois Driver’s License and automobile insurance also required
Comfortable conducting home visits
