Produces weekly, monthly, quarterly, and annual comparison QA reports to Care Management leadership
Supports and meets data needs for development and implementation of strategic initiatives to improve and enhance performance and services
Assists Care Management to meet requests for inter and intra-departmental information
Continually analyze new and revise specifications department metrics and effectiveness of processes
Assess and validate departmental benchmarks and KPIs including, but not limited to, transitional care programs, discharge planning activities, managed care insurance distribution, length of stay, avoidable days, and readmission rate
Submit and manage departmental IT requests for quality improvement activities including tickets and reports. Participate in monthly meetings with IT to discuss ticket activity, deadlines, and updates.
Identify and develop data collection methods using available software systems (Excel, Access, Crystal Reporting, Cerner DA2, Vizient database, Tableau)
Work with clinical staff to enhance clinical data capture and analysis, in support of internal and external quality improvement and reporting efforts and mandates
Educate Care Management leadership regarding potential impact on workflow requirements.
Perform gap analysis to identify areas that could benefit from workflow improvement activities and better data capture. Participate in and/or manage collaborative projects with IT and relevant clinical staff to develop mechanisms for capturing these data and workflow elements.
Perform hybrid medical record review of cases, validation, outlier cases drill-down, and other clinical data abstraction activities as necessary to support external quality reporting and quality improvement activities.
Provide guidance and support on clinical and statistical topics to Care Management leadership and other analytical and clinical departments.
Generate ad-hoc reports and perform ad-hoc responsibilities as deemed appropriate by Supervisor.
Maintain awareness of institutional quality priorities and work collaboratively with colleagues in Care Management, IT, and hospital leadership to accomplish organizational goals
Required Qualifications: Bachelor's Degree with Project management skills and the ability to work independently as well as collaboratively with diverse populations of healthcare workers is critical. Basic computer skills including spreadsheet and word processing are also required. Demonstrated ability to grow and adapt in a changing environment. Superior organization skills, detail oriented, and ability to be persistent and follow through.
Preferred Qualifications: Master's Degree in health related field or MBA. Clinical chart review and abstraction experience and ability. Intermediate experience with Excel. Prior experience with health care quality reporting and/or interpreting health care regulatory/accreditation requirements. Skill in presenting information to a variety of audiences. Experience in collaborative design of electronic health record components. Relevant experience may be considered in lieu of these qualifications.
