This position requires a nursing/clinical background. The Kidney Transplant Coordinator position is an out-patient, case management, office-based role in Medical Arts Pavilion 2. Weekends, Holidays and On-Call responsibilities per the guidelines of the department.
Christiana Care is seeking qualified candidates for the Kidney Transplant Coordinator opportunity that is a critical role for the Kidney Transplant Program . The Kidney Transplant Coordinator serves as a liaison between the transplant candidate, hospital personnel, physicians, ancillary services and dialysis / nephrology facilities to help accomplish successful kidney transplant.
The primary responsibility will be handling the coordinating of all clinical aspects of care, as part of the multidisciplinary team, and will facilitate and coordinate patient evaluation and UNOS listing of potential candidates.
Additional responsibilities include:
Identifying transplant candidates upon referral and interacting with appropriate staff to initiate evaluation process,
Coordinating evaluation as well as pre and post-transplant care with other team members including but not limited to social worker, pharmacist, dietitian and financial coordinator,
Assuring performance of necessary tests and studies to determine patients' candidacy for transplant,
Shared on-call responsibilities with other transplant coordinators (weeknights & weekends; remote; e.g. from home).
Monitoring and following all pre and post-transplant diagnostic studies,
Evaluating patients' health status on a regular basis,
Assume a resource role regarding transplantation for all inpatient staff nurses, and
Communicating all patient issues and concerns to appropriate transplant physicians.
Education & Requirements:
BSN in Nursing or other licensed clinician in a related field and/or equivalent combination of education and experience in surgical or medical specialty management.
A minimum of five years of related experience in health care, with a organ transplant experience preferred
