Provides comprehensive, patient and family focused care coordination for medically complex pediatric patients across the continuum of care as a member of the Coordination of Care Team. Collaborates with all team members to promote patient and family self management and to optimize the health and well being of the patient and family.
Bachelors required. Degree in Nursing

4 years pediatric nursing

Interfaces directly with physicians, patients, family members, and other health team members.
