Under the supervision of the Clinical Manager/RN, performs the primary function of LPN/LVN in the care of patients in the Advanced Wound Center (AWC). The Wound Center Nurse, LVN/LPN, is responsible for collaborating with the Clinical Manager, Physician and/or Program Director to coordinate services in providing quality care to patients with chronic non-healing wounds through the use of hospital and nursing standards. Essential Job Functions: Performs follow-up nursing assessment of patient's condition and reports findings to Clinical Manager/RN. Ability to cover in HBO supervision 3-4 days per week if needed Records wound assessment data accurately and communicates pertinent information. Assess compliance to the Clinical Pathway and communicate to Physician and Clinical Manager/RN. Identifies patient care situations that require intervention; implements and evaluates actions with Clinical Manager/RN. Identifies patient/family education and discharge needs and implements a teaching plan of care with Clinical Manager/RN. Collaborates with Physician and Clinical Manager/RN to develop a Multidisciplinary Plan of Care. Revise the plan of care and continuously reassess and evaluate until desired outcome is obtained. Formulates with Clinical Manager/RN the desired outcomes that are specific and measurable within a certain time frame and consistent with expectations. Complete a 5 day- 40 hour Introductory Course in Hyperbaric Medicine for certification Qualifications: Current CA licensure, Licensed Practical Nurse or Licensed Vocational Nurse (LPN/LVN). Preferred Wound Care Experience Preferred HBO experience BLS Required Minimum of 2 years medical/surgical experience; wound care experience preferred.
