Senior Social Worker

PRIMARY FUNCTION: To provide comprehensive mental health care for children and their families in the area of prevention and treatment in collaboration with the School-Based Health Center, school staff and community providers. Provides short term, goal-oriented counseling and ongoing support services to children and families. Applies knowledge of social work principles, concepts, methods and techniques in making a care plan and in formulating goals with adolescents. Oversees clinical screening for risk taking behavior and mental health problems for students in the SBHC Conducts student needs assessments and develops/monitors appropriate treatment plan and provides crisis intervention as needed. Sees 4 patients a day when students are in session. Provides support and linkages to school, Christiana Care Health Services (CCHS) programming and community services. Creates an environment that encourages and invites family involvement while respecting the students' confidentiality rights. Participates in SBHC continuous performance improvement activities. Maintains and updates student charts per CCHS policy and procedures as well as third-party billing guidelines. Develops and facilitates preventative educational opportunities for youth. Participates in site, SBHC, CCHS provider meetings. Supervises social work students as appropriate. Exhibits leadership and management in own practice within the SBHC team. Provides chaperone coverage for medical provider as outlined in CCHS Chaperone Policy- Outpatient. Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards or safety violations that may cause injury to oneself, other employees, patients and visitors. Performs other related duties as required. Must be a Licensed Clinical Social Worker (LCSW) or a Licensed Professional Counselor Mental Health (LPC MH) Two years in community programming with a childhood focus. KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS: Ability to function as a leader and member of an interdisciplinary team. Ability to relate to people like and different from self. Ability to organize and prioritize work assignments Ability to utilize good judgment, decision making and problem solving skills. Skill in collaborating with community and government agencies Strong working knowledge of community resources, state and federal programs Strong computer skills and working knowledge of Word, Access and Excel Strong working knowledge of Diagnostic and Statistical Manual (DSM) and ICD Codes Knowledge and clinical skills in adolescent care. Knowledge of individual group and family therapy. Knowledge of Social Work Theory and understanding and acceptance of Social Work Code of Ethics. Knowledge of Crisis Intervention Theory. Ability to learn and understand medical terminology. Ability to consistently present clear professional identity and purpose. Ability to demonstrate effective listening, speaking and written skills. Bilingual preferred SPECIAL REQUIREMENTS: Licensed in the State of Delaware
