Facilitates and coordinates the orientation and staff development programs for the case management staff. Participates in development of leadership skills in case management staff. Validates appropriateness of case management through both indirect and direct involvement in the clinical area. Responsible for coordination and facilitation of appropriate functioning of case management committees.
Qualifications EDUCATION : BSN and FL RN License required. EXPERIENCE : Experience with utilization review, including admission and discharge criteria and discharge planning preferred. Minimum of two years clinical experience.Knowledge of Centers for Medicare and Medicaid Services (CMS), Medicare, Medicaid, HMO and Department of Children and Families, community resources, discharge planning, extended care and rehab facilities and durable medical equipment.
Additional qualifications COMPETENCY: Promotes quality in day to day case management by monitoring, evaluating, planning coverage for identified case management needs. Facilitates case management program, including: policy/procedure revision and development, staff recruitment and retention. Coordinates performance improvement activities of RN Case Management staff. Conducts/factilitates orientation and development ofnew and existing case management staff. JOB KNOWLEDGE: Knowledge of clnical pathways, resource management, community resources, interviewing techniques, medical terminology, written and verbal communication skills, leadership skills.
ADDL CONSIDERATIONS: Well developed communication and organizational skills required, with the ability to problem solve and facilitate change effectively.
