Cook Children's Health Plan is looking for a Case Manager/Service Coordinator, RN. You would performs case management/service coordination activities that enable Cook Children's Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through:
This is an office-based position with required field work. You would be required to drive and perform in home visits up to 75% of the time. Some weekends may be required to meet member needs.
Education and Experience:
Requires a Nursing Diploma or Associates Degree in nursing. Bachelor of Science in nursing strongly preferred
Three years of clinical experience
Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions preferred.
Strong skills in medical assessment/medical record review and/or behavioral health assessment. Prior MCO experience a plus
Must possess the ability to function in an interdisciplinary, team environment
Self-starter with the ability to be effective in a large, complex and highly matrixed organization is a must
Need to possess the ability to focus on both the details and the "big picture" simultaneously
Bilingual English and Spanish a plus.
Effective written and verbal communication skills that allow clear and concise presentation of conclusions and recommendations to a variety of audiences
Critical thinking and problem-solving, ability to plan and organize effectively, proficient in typing and in use of computer applications (Excel and Outlook)
