This position is responsible for the delivery of a high level of service and associated quality to individuals requesting outpatient subspecialty service from any department or provider. The primary functions include:
1. Triage all inquiries/requests from patients/parents and referring physicians (and their office staff) for complex appointments (via phone, email, in-basket orders or messages).
2. Coordinate care and provide guidance and education to the family to ensure the patient receives the necessary care.
3. Register and schedule or facilitate scheduling of these patients with the appropriate providers.
4. Address clinical and non-clinical inquiries according to established divisional protocols and guidelines.
5. Improve patient’s access to care by assuring efficient and effective coordination of patient appointments.
Receive intake from patients/parents, families, physician office representative, and schools for triage, assess urgency for evaluation and management of care for outpatient appointments. Provide clinical assessment expertise to scheduling parameters – timing of visit, appropriate provider, etc. Collaborate with clinical provider as necessary.
Serve as an educator/resource to the Patient Navigation Team on divisional protocols for outpatient appointment scheduling, patient triage, provider expertise, and other details which are pertinent to accurate appointment scheduling.
Participate and represent Patient Navigation Team in department programs and meetings.
Communicates (by preferred method) all procedure related preparation information to families directly during the scheduling process.
Track patients and facilitate coordination and continuity of care.
Work collaboratively with division members relative to maintenance and upkeep of current divisional scheduling protocols.
When necessary, create correspondence in response to a patient request for clinical provider review.
Serves as a liaison for communicating patient care plans between all stakeholders (i.e. PCP, Specialists, Families, etc).
Proactively develops and enhances processes with the goal of improving the clinical experience for referred patients and the referring physician.
Maintain data and related outcomes to complex scheduling.

Current, unrestricted DE Registered Nurse license upon hire
PA and NJ Registered Nurse Licenses to be obtained within 90 days of hire
