Your Role: This clinical pharmacist will provide telephonic medication management services to patients who were recently discharged from Vanderbilt University Medical Center. The primary responsibility of this pharmacist is to provide support to patients and families and assure appropriate, safe, and cost-effective drug therapy during transitions of care. The pharmacist in this role will interact patients remotely to provide medication reconciliation, patient counseling, disease management, and helping patient to access medications after discharge. Other responsibilities may include process and quality improvement and other duties as assigned. Seeking candidates who are problem solvers with an innovative spirit and a positive attitude. Exemplary customer service experience, effective communication skills, and ability to work with a multidisciplinary team are needed to be successful in this role. Position Shift: This position will work 10 hour shifts with 7 days on, and 7 days off, including holidays and staffing every other weekend.
