We value a patient-centered, gender-specific philosophy and provide a range of services that include primary care, Gynecology, Mental Health, Maternity Care, Mammography, Pelvic Floor Therapy, Sexual Health, IVF Treatment, Urgent care, Hormone Replacement Therapy, and many more. As a Women's Health Care Coordinator, you will work in collaboration with the Women Veterans Health Care interdisciplinary team, to provide primary health care services for female veterans. You will be accountable for adhering to established standards of care including; clinical guidelines, policies and procedures. The main goal is to coordinate care to improve efficiency and timeliness with patient access to care. Duties include but not limited to: Coordinating and tracking gender-specific care (i.e. maternity care, breast care/mammography, gynecological coordination, etc.) of patients throughout all aspects of the medical center, including Community Based Outpatient Clinics, as well as community vendors and/or agencies. Assisting with initial VA consult referrals for gynecology, mammography, non-VA maternity care, and/or other care, assisting with VA or community services for the patient with an appropriate provider, to include coordinating any subsequent appointments or follow-up, as needed. Educating patients on a variety of gender-specific topics, to include importance of routine medical screenings, and authorized VA and community-based resources for maternal, obstetrical, and infant care. Contacting pregnant veterans on at least a bi-monthly basis throughout the duration of pregnancy to assess clinical and/or social care needs, facilitating care and services between VA and non-VA care providers. Assisting veterans who have presented to VA or non-VA emergency department for gender-specific emergencies (i.e. miscarriage) to secure appropriate follow-up care such as lab, ultrasound, emotional support, and/or other care as needed. Coordinating care with the Veterans PACT. Ensuring seamless and timely re-entry into Primary Care following delivery by assisting with a follow-up appointment within 90 days of infant delivery. Documenting patient care interactions accurately, succinctly, and timely, using appropriate notes, templates, and clinical reminders, in the electronic medical record. Utilizing and maintaining tracking spreadsheets and systems as required by VHA Directives and Handbooks. Managing appropriate use of the electronic alert (flag) for pregnancy/lactation and/or other issues in the electronic medical record. Understanding and responding to mammography reports, BIRADS, Pap, and other gender-specific screening/diagnostic test results. Communicating and documenting communication of test results for female specific testing. Contacting patients who fail to keep mammogram appointments, or who are delinquent in obtaining recommended breast cancer or other diagnostic screenings (i.e. Pap), to determine and resolve obstacles the veteran may have with keeping appointments. Maintaining professional skills in the application of professional bodies of knowledge and accepted clinical practices that will facilitate oversight of service delivery of patient care. Participating in quality improvement programs and initiatives including system redesign, to improve access, quality, efficiency, and patient satisfaction.
