You will be responsible for:
Reviews physician orders to assure accuracy and appropriateness, and intervenes with prescriber and documents as required.
Verifies orders in the hospital information system.
Performs renal dosing adjustments, IV to PO conversion, therapeutic interchanges and other medical staff-authorized policies and protocols.
Provides clinical consult services for parenteral nutrition, pharmacokinetics, and anticoagulation management.
Dispenses and/or compounds medications, sterile products and chemotherapy.
Supervises the work of pharmacy technicians and pharmacy interns.
Monitors for, and reports medication errors and adverse drug reactions.
What you will need:
Minimum of Bachelor of Science degree in Pharmacy from an accredited college of pharmacy. Doctor of Pharmacy preferred.
Current licensure with Florida State Board of Pharmacy as a registered pharmacist.
IV to PO medication conversion expertise.
Pharmacokinetic dosing and monitoring expertise.
Anticoagulation dosing and monitoring expertise.
Hospital pharmacy experience.
The pharmacist supports the hospital mission and pharmacy department through the practice of the profession of pharmacy, preventing, identifying and resolving drug-related problems to ensure patient safety, improve patient satisfaction and minimize costs.
