The Patient Data Coordinator is responsible for the collection and input of patient sensitive quality data for ImproveCareNow measures/initiatives from the EHR. This includes reviewing all enrolled ImproveCareNow (ICN) patient medical records; identifying and abstracting required data from the medical record and entering collected information into ICN database. This position will work closely with the GI Clinical Coordinator on all aspects of data handling/management and any project related to ICN and other assigned projects. This position is also responsible for scheduling appointments/procedures for the GI Motility Program, Quality chart audits, and management of Title XIX paperwork to ensure completion.
High School graduate or equivalent required
Minimum of 5 years of related Medical experience
Knowledge of EHR documentation and medical terminology is required
Proficient in Microsoft office products and data entry experience required
Must be highly detail oriented, have the ability to remain focused, have good organizational skill, and excellent oral and written communication skills
