Overview

St. Luke's Health System in Boise is seeking a Nurse Navigator to join our Capital City Family Medicine team.

The clinic sees people of all ages, through all phases of life, whether it's for preventive care or treatment for a specific condition or acute illness. The clinic is dedicated to improving health and experiences for children, adolescents, and young adults with differences in sexual development (DSD) and/or gender identity. Our multidisciplinary team is welcoming and compassionate, providing integrated health care in a safe and affirming atmosphere. We offer same day appointments as well as scheduled appointments. We promote the concept of a medical home, emphasizing a continuous relationship with a provider coordinating care for both wellness and illness.

The Nurse Navigator will assist with outreach to current pt panel, In Basket support, referral resources, intake, relationship building with community resources, direct point of contact, education, and more.

Schedule: 4, 9 hour shift, day hours. Variable day off.

Responsibilities

• Serves as a single point of contact for patients undergoing treatment,.
• Serves as a liaison between the patient and their family and the patient's network of care providers.
• Educates patient on diagnosis, treatment options, and prognosis.
• Assists the patient in scheduling appointments, completing paperwork, or securing referrals.
• Facilitates short term and/or long term interventions.
• Possesses comprehensive knowledge of subject matter.
• Handles complex issues and problems, and refers only the most complex issues to higher-level staff.
• Other duties and responsibilities as assigned.

Qualifications

• Education: Bachelors of Nursing Degree
• Experience: 2 years's of RN experience
• Licenses/Certifications: Current RN License in the State of Practice

Preferred Qualifications:

• Previous experience in behavior health or psychatric setting
